 Abstract: Rape is an offence that causes tremendous suffering for the victims and harms the society in general. A widespread assumption about rape is that it is caused by a deviation in sexuality. Although some rape is the result of deviant sexual interest, a significant part of it is not. A more frequent occurrence among rapists is an antisocial personality disorder (ASPD), where the offence can represent a result of aggression or an uncontrollable impulse. Rapists with ASPD are a subgroup which represents a critical part of rapists and also presents clinicians with a significant challenge due to their special treatment needs and responsivity issues. In addition they are in a group with the highest recidivism risk. Despite the groups’ impact, development of treatment programs for intervening with this population is still in its infancy.
The current tendency in sexual offender treatment is to treat different types of sexual offenders within the same program. These programs can be criticized for taking a child molesters perspective and predominantly meet their treatment needs. It can be considered as problematic because of essential differences between child molesters and rapists with ASPD. Particularly in relation to treatment readiness, therapeutic alliance capability, cognitive distortions, empathy deficits, deviant arousal and antisocial behaviors all of which can have implications for treatment outcome.
This thesis takes a critical stance for a homogeneous offender treatment approach because of the assumptions that it will not have the ability to capture the group’s treatment needs and therefore neither reduces recidivism risk. In an attempt to reach effective treatment for rapists with ASPD, there will be presented and discussed three types of treatment program, which have in common that they proceed from the specific treatment needs and responsivity issues that characterize rapists with ASPD. Focus on differentiated treatment programs specific for rapists, treatment services for high risk offenders with personality disorder and violence reduction program. Overall, treatment for this group is very challenging. It may be effective but has to be tailored to individually identify difficulties and needs, by targeting the treatment approach.
An explicit aim in this thesis is to shed light and facilitate knowledge in a group which can be considered as marginalized. This group also deserves more attention and adequate treatment, where reduction of rape victims is the primary objection.
